All Health Forms for the 2026-27 school year must be completed by Monday, August 3. The forms are available on the BE WELL Health Portal on the Bernard Zell Health Office page. After logging in, click on the button that says "Requests" to find your child(ren)'s forms.

You will have access to the BE WELL Health Portal and our Parent Portal (myBZ) starting Wednesday, July 15. On July 15, you will receive an email from Veracross (Subject: myBZ) with detailed login instructions.
