Bernard Zell is pleased to announce the confirmation of our two on-site vaccine clinics open to all eligible students, parents, employees and non-student siblings taking place on Friday, November 5, 2021, from 7:00 a.m. - 12:00 p.m. and Friday, December 3, 2021 from 7:00 a.m. - 12:00 p.m.
In partnership with UChicago Health, eligible individuals will be able to opt-in to receive their initial series of COVID-19 vaccine doses, a booster dose as well as a flu vaccine. While vaccination is not required, it is highly encouraged for the continued health and safety of our community.
Registration will open for parents on Friday, October 22 at 9:00 a.m. and is a two-step process that will route through the Parent Portal on BZConnect. First, parents/guardians will need to complete a registration form with all pertinent information for each individual getting vaccinated. Second, they will select an appointment time slot(s) on sign-up.com. Both the registration form and appointment sign-up are required to complete registration. A vaccine clinic email with sign-up links and additional information will be sent to all families before registration opens on Friday morning.
The clinic will be dependent upon the Food and Drug Administration (FDA) approval of the Pfizer-BioNTech COVID-19 vaccine for children ages 5 to 11.
The following vaccines will be available to eligible individuals:
Pfizer-BioNTech COVID-19 vaccine for ages 5-11* and 12+ (including boosters)
Moderna COVID-19 vaccine for ages 18+ (including boosters)
Johnson & Johnson COVID-19 vaccine for ages 18+ (including boosters)
Flu vaccines (injection or nasal spray) for ages 6 months+
*Availability will be dependent on FDA approval
Insurance information will be collected at the time of registration and is required for flu vaccinations and requested (though not required) for COVID-19 vaccines. No insurance is necessary for COVID-19 vaccines. HMOs are not accepted, most other insurance is accepted.
Any student under age 12 will need to have a trusted adult on-site at the time of vaccination and for the 15 minutes following. Trusted adults can be the parent, guardian, grandparent, nanny or family friend. Teachers will not be available to fill this role.
Registration for the second clinic will be announced after the first clinic concludes.
